KEY TO MY ART MEDIA Waiver/Agreement

I acknowledge and understand that Key to My Art may periodically photograph or video its participants and their artwork and I grant my full permission for Key to My Art to photograph, video, record, or reproduce images of me and/or my child and/or my group and their artwork. In the case that I do not agree to this, I will submit, prior to the class or event I or my child/group is attending, in writing, a letter stating that I do not agree.
